With this questionnaire you can gain an overview of how far your practice is already on the way to fulfilling the requirements for the Praxis Vielfalt Quality Seal. The result of the self-assessment is only available to you. It is for orientation only and can help you decide whether or not you would like to participate in Praxis Vielfalt Gütesiegel.
It is best to use the instrument in small groups. Doctors and other practice employees can work together. Depending on the need for discussion, the instrument takes about 45 minutes to use.
The answers are not about right or wrong. Discuss each question briefly with your colleagues and then decide whether the statement is more or less correct for your practice.
The questions answered with "rather YES" point to areas in which practice is already relatively far advanced in fulfilling the requirements for the quality label.
The questions answered with "rather NO" point to areas for which the acquisition of the quality label can be particularly valuable.
Once you have answered all the questions, the results are automatically evaluated and summarized.
You can print this evaluation. If you decide to participate in the seal of quality, you can repeat the self-assessment at any time and compare it with your previous result.
Now use the evaluation as a basis for a team discussion in which you consider your possible participation in the quality seal process with all participants.
